Seems it works differently depending on the attention level of enemies. Even with Legendary Camo if i use it while fighting enemies (so they are already in alert/combat mode) they will still see me but their fire rate is lower and they act more stupid. So more of a "Camo" Effect than real invisibility which makes sense.

There are two perks related to Optical Camo in Cool. As far as I understand they both work having the Optical Camo cyberware. One gives you bonuses for taking down an enemy while using it, the other activates Optical Camo automatically when crouching but only drain its duration when sprinting or sliding, this one pretty cool tbh xDI looked online but can't really believe it this is actually in game and not fixed yet. Apparently once you reach headhunter level 40 your optical camo always activate when you grapple an enemy, cool little nifty upgrade. Except it still uses your charge, and because its a skill that automatically level up, you can't turn it off or avoid picking it up.There's a mod to fix this if you haven't found it already.
